¨Ian and Linda¨

Ian and Linda live in a condo, and enjoy a happy life with hobbies, time with adult children and grandchildren, and a strong connection with their community. When Ian had a stroke and spent time in the hospital, he wanted to go home rather than a rehab facility but his  inpatient care team would not discharge him to home without additional support.  

 

Ian and Linda hired In-Home Partners to ensure his physical safety while rebuilding his strength. 

We were in their home from 7:30am-10pm, allowing Ian to resume his normal daily schedule. Expanding upon the exercises prescribed by the home health physical therapist, Ian saw a quick and impressive recovery from his stroke symptoms as we accelerated his progress with constant exercise and reinforcement. 

 

Linda’s stress level dropped significantly once she saw the support Ian was getting. Because we advocated with his health care providers, we could expedite necessary services, allowing Ian not only a swift recovery from his stroke challenges, but a better level of mobility and strength than before his stroke. 

 

We now work regularly with Linda and Ian. There are neighborhood walks, tailored strength training, and ongoing health care advocacy. Additionally, we provide 24/7 emergency support, so they know we will have all their needs met should an emergency arise, that we will regularly update their families, and that their home will be attended to with care and respect. 

 

After becoming familiar with everything we can do, Ian and Linda have benefited from research support in finding a new primary care provider, medication management support, repotting overgrown houseplants, changing high lightbulbs, and organizing advance care planning documents. There is very little we can’t do to help clients feel safe, organized, and cared for!

¨Helen and Kevin¨

Helen and Kevin live in a condo, and are in great health despite Kevin’s early Alzheimer’s diagnosis. Helen hired IHP to increase the safety of their home in case of accident or emergency. 

We installed three smart speakers, with clear instructions visible to Kevin about how to activate the speaker to contact them in case of an emergency. They know how to verbally use any of their smart speakers to activate their IHP emergency contact, or emergency services if needed. 

 

We also remedied a few technological challenges with their home computer, installed several 

subtle and effective safety measures around the house, explained their health care coverage and even help them prepare for a big trip to Michigan!

¨Diane¨

Diane recently turned 65 and became eligible for Medicare. The process for applying for Medicare and knowing which plan was best was daunting for Diane, so she activated IHP’s insurance support. As an experienced health plan executive, Alison has been well-poised to navigate this confusing process at Diane’s side. She has scheduled meetings and accompanied Diane to them, and has also reviewed all her healthcare-related mail and identified actions Diane needs to take. Diane now has a Medicare plan that she understands, and that meets her health care needs.

¨Carol¨

Carol lives alone and is in good health despite a few falls over the past few years. She is close with her extended family, though none live nearby. Carol enlisted IHP to increase the safety of her home before eventually moving into a continuing care retirement community. We conducted a safety assessment, and made several recommendations about reducing fall risks, stabilizing precarious pieces of art, and reinforcing floor coverings with better grip. We also installed a safety device that Carol can verbally activate if she’s had an emergency and cannot get to her phone. Finally, we coached her on how to utilize her smart watch if she ever has an emergency in the future and is not near her phone, or cannot verbalize a need for help.
