Joining you for a Portland medical appointment. We will: 

  • identify your goals before appointment

  • take notes for you

  • remind you of important questions

  • ensure all your needs are met by your providers

  • arrange transportation for an additional cost

 

Providing discharge support. We will: 

  • be the safe person to accompany you after a medical procedure

  • remain with you after discharge until you feel safe 

 

Funeral/memorial service planning. We will: 

  • provide you with several funeral home suggestions

  • help you pre-plan and pre-pay for services 

  • accompany you to planning appointments

  • ensure your future needs are met without feeling pressured

  • retain a copy of your plan for you

 

Overnight stays after hospital discharge 

  • Costs determined on a case-by-case basis 

 

Caregiver support 

  • 4-hour windows of time supporting your loved one with Alzheimer’s or a dementia diagnosis

  • Support provided in your home or neighborhood

  • Attend to tasks that are otherwise a challenge: schedule a haircut, get a massage, visit with a friend, take a nap, or anything else you put off in order to care for your loved one

 

Life history/family story: Your Personalized Story

  • Spend time with IHP sharing the stories that mean the most to you and your family with a bound book as the result

  • Create memorable keepsakes for you and your family to share your favorite memories

  • Include special family pictures, which will be handled with extreme care and respect

  • We’ll do all the work – all you need to do is chat with us! 

  • Up to 10 hours in-person interviews to capture what means the most to you
